About the use of non-contract examination institutions
- Even if you take health check in non-contracted hospital you are able to apply for a subsidy.
- You can apply for subsidy requests for the use of contracted medical examination institutions or non-contracted medical examination institutions once a year.

When you have all the necessary documents, please submit them to the charge department of the company.※If you wish to continue voluntarily, please send it directly to the Health Insurance Association.
【Deadline of submit】
Please apply during the fiscal year (April 1st to March 31st of the following year).《Precautions》
- If the submitted documents are not complete, the subsidy will not be paid.
- If you receive medical treatment under insurance (30% of copayment), you will not be eligible for subsidy.
《Precautions when attaching a receipt》
- For those over 40 years old, please attach a receipt with a breakdown of the specific medical examination fee.
